“Filler” gets used as a catch-all term, but not all fillers work the same way, and not all of them are even technically fillers in the traditional sense. Two of the most common questions we get in consults are what makes Radiesse different from something like Juvederm, and why we sometimes recommend one over the other. Here’s the real breakdown.

Traditional Dermal Fillers (Hyaluronic Acid)

Products like Juvederm, RHA, and Belotero are all hyaluronic acid, or HA, fillers. HA is a substance your body already produces naturally, and these fillers work by adding volume directly to the treated area, smoothing lines, restoring lost fullness, or enhancing features like the lips or cheeks.

HA fillers give immediate, visible results, and each brand and formulation is designed with a specific purpose in mind. That’s why we don’t reach for the same product on every patient. We choose between Juvederm, RHA, Belotero, and others depending on your anatomy, the area being treated, and the look you’re going for. Another major benefit of HA fillers is that they’re reversible. If needed, we can dissolve them with an enzyme called hyaluronidase.

Radiesse: A Different Category Entirely

Radiesse isn’t an HA filler. It’s made from calcium hydroxylapatite, or CaHA, a mineral-based compound that behaves very differently in the skin. Used in its standard, undiluted form, Radiesse provides immediate volume, similar to an HA filler, but it also stimulates your body to produce its own collagen over time. That means the results can actually continue to improve even after the product itself has broken down.

Hyperdiluted Radiesse: Our Favorite Tool for Skin Quality

Here’s where it gets really interesting. When Radiesse is diluted, or hyperdiluted, with saline or a numbing agent, it stops acting like a traditional volumizing filler and instead becomes a powerful biostimulator. Instead of adding bulk, hyperdiluted Radiesse gets to work immediately stimulating new collagen and elastin production, which improves skin quality, tightens lax skin, and restores a smoother, firmer appearance from within.

We use hyperdiluted Radiesse in just about every way possible in our practice, not just on the face, but on the neck, décolletage, hands, and other areas where skin has started to thin, crepe, or lose its bounce. It’s one of our favorite tools for patients who aren’t necessarily looking for more volume, but want their skin to look and feel healthier, tighter, and more youthful over time.

The Key Differences

• Composition: HA fillers (Juvederm, RHA, Belotero) are made of hyaluronic acid. Radiesse is made of calcium hydroxylapatite.
• How they work: HA fillers primarily add volume. Radiesse can add volume and stimulate your own collagen production, and in its hyperdiluted form, it skips volumizing altogether to focus purely on skin quality.
• Reversibility: HA fillers can be dissolved with hyaluronidase if needed. Radiesse cannot be reversed the same way.
• Longevity: HA fillers generally last 6 to 12 months depending on the product and area. Radiesse’s collagen-stimulating effects can continue well beyond the time the product itself is metabolized.
• Best for: HA fillers are ideal when you want immediate, precise volume, think lips, cheeks, or smoothing specific lines. Hyperdiluted Radiesse is ideal when your main concern is overall skin quality, laxity, and texture rather than added volume.
